Troop 41 Forms

Class 1 and 2 Medical Form is the standard medical form needed for all scouts.

Class 3 Medical Form is for scouts going on an event over 5 days (such as summer camp) or high adventure, and also for adults over 40.

Permission Slip This is a generic permission slip for when you cannot find the one your scout brought home.

Parental Consent Agreement is needed for each scout giving him perimssion to participate in Troop activities.

Photo Release gives the Troop permission to use photos of the scout.
