Certified copies of Birth and Death Certificates can be requested for individuals who were born or who died in Mountain Lakes.  To request a certified copy, use the form here.  This is a multi-purpose form used by the Borough for any request for public records.  When you fill it out, please be sure to indicate exactly what you are requesting.  Fax, mail, or bring it in to Borough Hall.  If you mail it, please be sure to include the required fees. If you fax or bring it in, you will pay the fees when you pick up the documents.
