This is a multi-purpose form used by the Borough for any request for public records. When you fill it out, please be sure to indicate exactly what you are requesting. Fax, mail, or bring it in to Borough Hall. If you mail it, please be sure to include the required fees. If you fax or bring it in, you will pay the fees when you pick up the documents.

The terms "public record" and "government record" in New Jersey do not include:
- Criminal investigatory records
- Victim’s records
- Inter-agency or intra-agency advisory, consultative, or deliberative material
- Emergency or security information or procedures for buildings and facilities
- Administrative or technical information regarding computer hardware, software, and networks which, if disclosed would jeopardize computer security
- Information regarding labor-management negotiations including statements of strategy or negotiating position
